Chautauqua Plan Administrators, Inc. (CPA) is a third party administration firm located in western NYS. CPA provides personalized service to firms interested in implementing defined contribution (DC) plans as well as the subsequent administration of the plan. We have a prototype DC plan document complete with the plan document, summary plan description and administrative forms. We prepare all government forms including Form 945 and 5500. We administer plans with as few as three participants and as many as 850. CPA provides assistance with 'sign-up' and follow-up educational participant meetings and attends investment committee and Trustee meetings.
